Michelle was angry with young Parker Tyler Griffin because he was a boy, when she wanted a girl. She constantly abused him and forced him to dress up as a girl.

In this guise, he kidnapped and murdered 15 young women. Adult Josh finds Dalton and starts to look for a way out of the Further, guided by the voice of his wife Renai Rose Byrne. Along the way, he meets up with the old woman and confronts her, telling her to get away from him. As she reaches out to him, Josh awakens from his projection and Dalton from his coma. Later, while Dalton pigs out in the kitchen eating spaghetti, Josh gathers up the old photos of himself, intending to dispose of them.

As he hands them to Elise, Elise looks at his hands and sees that they are gnarled and wrinkly, like an old woman's hands. She quickly snaps a picture of Josh with her digital camera, causing Josh to go into a rage and strangle her.

Hearing Josh screaming, Renai goes to see what's wrong. She is horrified to find Elise dead. Renai looks into Elise's camera and sees a photo of the old woman possessing Josh's body. In the final scene, Josh is heard saying to Renai, "I'm right here.

With "Josh" possessed by Parker Crane in pursuit of Renai and the boys, who are hiding in the basement, Dalton volunteers to go into the Further to find the real Josh. Taking his end of the tincan telephone with him, Dalton astral projects into the darkness. Meanwhile the real Josh, trapped in the Further, encounters Parker's victims standing together with sheets over their heads. He begins to remove the sheets, but one of the victims turns out to be Michelle Crane, who leaps at him, eager for the kill.

Fortunately, Elise and Carl catch up to Josh, and Elise stops Michelle by hitting her repeatedly with a rocking horse. Back in the real world, "Josh" has broken through the basement door and is about to kill Renai when he notices Dalton "sleeping" in a corner. Back in the Further, Elise orders Josh to return to the real world and to take Carl with him because Carl isn't really dead she heard his heartbeat when she hugged him.

Josh and Carl begin to leave, come across Dalton, and use the string from his telephone to guide them back into the basement. Astral Josh re-enters his body and proves it to Renai by admitting that it was he trying to play her song on the piano. Later, Josh and Dalton voluntarily get the memories of their astral projection abilities erased by Carl. In the final scene, Specs and Tucker arrive at a house where Elise has told them a young girl named Allison Dannay Rodriguez needs their help. The family's younger daughter notices Elise standing behind Specs and Tucker, but no one else can see her.

When Carl summons Elise using his dice to communicate with her, she viciously fools him and the rest of the group, in pretending to be the deceased psychic and drives them into a trap when her clues, first lead them to the hospital Lorraine used to work at and treat her son as a patient, then to what used to be her and her son's home. There she reveals her true identity through the dice, calling herself 'Mater Mortis' Mother of Death and attempts to kill Carl and Lorraine by dropping a chandelier from the ceiling onto them. During Specs and Tucker's inspection of Parker Crane's childhood's room as well as Josh's wander into the Further, it is revealed that she was purely insane and violent toward her son as she forced the young Parker into assuming the identity of a girl, and insisted that his name was Marilyn. In a re-enacting of one of Parker's memories of her, she is seen to slap him really hard across the face as he fails to remember this when signing a drawing he made for her with his real name. Through messing up her son's identity and mentally breaking him down, she managed to force him into a lifetime of serial killing of mostly women as it appears when the ghosts of his victims are unveiled. At the end of the movie, as she tries to kill Josh, she and the memories Parker has of her are defeated when Elise repeatedly pounds her with a rocking horse and killed her. After her final death she presumably left The Further and passed on presumably to Hell. An elegant woman dressed in a long white gown haunts Lorraine's house.

I imagine that it might come as a surprise to some people that I chose this film for the next entry in Horror Queers. Its central villain is a man who dresses up like a woman and kills people before eventually being committed to a mental hospital for trying to castrate himself.
